Don't ask me.... his weapon was checked by the armoury techs
after the incident, and it was certified to be working fine.
Whatever the case, if you have IA, the first thing to do is to
unload, check clear your weapon, and to visually make sure that
there is no round in the chamber.
And most importantly.... when your weapon is loaded, it should only
be facing the front. Nowhere else.
